I confirm that I wish to revoke the measured charge notice I completed and revert back to an unmeasured charge. I understand that if I have a meter installed at a later date, the property will be permanently metered.

I also understand that in order for the property to be reverted to unmeasured charges, a final meter reading must be provided for the premises. I understand that the final reading will be taken by DCWW's reresentative when the meter is removed, however to avoid delays I can read the meter myself and provide a self-reading.

I confirm that the property remains a household premise, occupied as a single dwelling.
